Registered Students are eligible for care at the Student Health Center, located at 1800 Volunteer Boulevard. No health insurance is required and students pay minimal or no charges, depending on the services rendered.
UT pays for health insurance for qualified TAs/RAs. Spouses and dependents may be covered by Student Health Insurance, but are not eligible for treatment at the Student Health Center.
Health Forms
Complete and submit the Immunization Requirements/Record and Medical History Questionnarie forms prior to classes. The forms can be found at Immunization Requirements. Send the completed forms to:
